Windows 11 BSOD

Please note that the RAM currently states 32 GB, however this is typically 64 GB.  I have been pulling chips & swapping/conducting RAM checks as a mini DOE.

 

I am capable of giving quite a bit of feedback on what I have been experiencing, if required. The below is a high-level synopsis:

 

BSOD's are inconsistent in terms of the error type, and occur randomly.  Until recently, they would only occur after my initial morning startup, post-login, seemingly while launching/syncing MS Outlook or MS Teams.  Frequency was once or twice a week.  It has increased to every day, with boot failures now occurring before I'm even able to get to the Window's log-in screen.  All updates, both from Windows and from Asus are installed.  

 

Odd behaviour began from the onset of purchase, however a contingent of this was likely self-inflicted, as I downgraded to Windows 10 to be compatible when certain Engineering softwares.  Hardware incompatibility seemingly forced me to Windows 11, and at the onset of a clean install, things were OK, but as time passed, the aforementioned became more and more prevalent.

Below are a series of emails exchanged with my IT department documenting my issues and experiences.
I've removed any personal information and only included my personal submissions of the faults and BSOD's.

 

Note that today, November 22, 2023, BSOD's occurred repeatedly upon boot.  I could not even reach the log-in screen.  I performed a system recovery and was able to launch (which is why I was able to imitate this thread), however BSOD's are still occurring.  The last one was during a memory test, however I was working/adding stress while the memory test was in progress, which may have made it self inflicted.

Over the long weekend, I decided to bite the bullet and perform a fresh Window’s 11 install to try and counteract the occasional blue screens I’ve been receiving as of late (Justin is aware).  It took about 8 hours by the time I got all of my software reinstalled, but everything seemed to be sorted and working as intended as of last night.  I also took a Macrium Reflect image after the fresh install + my software installs so that I have a (presumed) clean restore point.

 

This morning, I came into the office and fired up my computer (plugged into my dock).  My monitors were a mess because I hadn’t yet configured anything, so I began to set my extended displays, primary monitor, etc.  While doing this, of course, I got another blue screen.  Unfortunately, I didn’t catch the error message.  I fired back up, and since then there have been no problems (similar M-O to my experiences prior to the clean install yesterday).

 

So, clearly a fresh install didn’t do the trick.  I’m now wondering if it’s something to do with the drivers for my dock?  Although, I’ve had blue screens before (prior to the fresh install) while at the race track and alike without the dock part of the equation what-so-ever…

 

The dock in question is a Visiontek unit (recommended by Anthony).  The drivers were downloaded from the official website to ensure I had the latest, here:

Logging another event –

 

Since we last spoke things have been flawless.  However, this morning upon initial startup (after windows boot/login, and me firing up Outlook), I received a ‘PFN List Corrupt’ blue screen message.  As per our last conversation I’m not sure there’s anything to be done (updates required?) however if there is something we can check, please let me know.  After some quick googling around, it sounds as though this may have been driven by an app/software, so if there is a way to identify what the trigger may have been, I’d be interested.

Logging another event –

This time it was “IRQL_Not_Less_Or_Equal”.

 

I’m starting to suspect that maybe it has something to do with Outlook.  The error message seems to change, but whenever I have a crash, it seems to be right after the computer boots up and I hit outlook to launch.  I get into Outlook and hit a mailbox or sub-folder, then the crash ensues.

 

This time, it relaunched me into the BIOS.  It wasn’t exactly a screen I recognized, either – Asus ROG EZ Flash Utility?

I ended up exiting and things ended up launching fine and behaving normally since.

Logging another blue-screen event –

This time it was “DPC_WATCHDOG_VIOLATION”.

 

The system restarted and went into that BIOS menu which I again exited and everything started up normally.

Once again, the crash happened after everything was booted up and I engaged MS Outlook.  Outlook booted, I started to get overlays and reminders about meetings, began clicking through my folders and the system froze – my extended monitors crashed and I could only see the main laptop screen.  I still had mouse function but couldn’t click anything.  After about a minute, the aforementioned blue screen appeared. 

 

I’ll look for updates in a moment, but I haven’t installed any new software nor have I done anything different to my knowledge. 

The blue screens are seemingly happening on their own with increasing intensity.

Another blue screen a moment ago (and another new error I haven’t seen before) – This time it was “Unexpected_Kernel_Mode_Trap”.

 

I was running on battery power in a board meeting.  The battery died and the machine went to sleep.  When I went back to my desk, I plugged in my USB-C dock + power and booted up.  Upon boot-up, the blue screen was received.  After some Googling, it sounds like this error may be hardware related, which leads me to wonder if it had something to do with my USB-C dock and the associated drivers.  We once thought as though my issues could be related to these devices, but I don’t think we were able to conclude anything. 

 

The dock is a VisionTek unit and the drivers are native to them (below).

Following up on the blue-screen investigation.  Did you uncover anything from the logs?

Things ran very smoothly this past week/weekend while in Atlanta, however this morning I received another blue screen – this time it was SYSTEM_SERVICE_EXCEPTION.

 

The crash occurred while trying to start Outlook.  Outlook would launch, but say (Not Responding).  After about 3 attempts to close and re-open (including in Outlook Safe Mode), the machine crashed and the blue screen appeared.  Upon restart, another blue screen appeared during boot up (right when it got to the log-in screen), and while the windows welcome jingle began to play, it glitched and gave a bizarre noise.  I couldn’t catch that error code because it happened too fast.

 

Again, I’m getting the impression that:

 

  • There’s some tie-in to Outlook in all of this – once booted, crashes seem to happen when I launch Outlook / a few seconds later when it’s syncing up.
  • Something with the driver for my multiscreen display (previous email), because these issues have not been happening as of late while at the track without my dock.

Just had another blue-screen incident.  “IRQL_Not_Less_Or_Equal”.

 

Same M-O as previous; PC booted up, I opened Outlook, and the crash occurred. 
Interestingly, this time upon re-start, my extended monitors wouldn’t function.  I checked for updated drivers (there was something), installed them, and still nothing.  I did a few restarts and again, nothing.  Finally, I rebooted the dock itself and they began to work again.  This hasn’t been something I’ve experienced before.

 

I’m growing more and more convinced that these problems are related to either Outlook or the dock drivers.

This said, my brother (with the same PC and dock), is not having these issues…

 

**NOTE** BSOD's HAVE ALSO OCCURED WHILE NOT PLUGGED INTO THE DOCK!

Just had another blue screen.  This time, it didn’t seem to be a function of Outlook, but rather, MS Teams.  I fired up outlook and looked at the sync progress.  It didn’t stipulate any specific folders, only ‘Updating XXXXXX’, etc.  This was all completed successfully – no crash.

 

I then went to fire up MS Teams (manually, I have auto boot turned off), and right as the window popped up, the blue screen crash occurred. 

I can’t say I’ve documented a circumstance like this to this point; it has always seemed to be Outlook related.  Although, perhaps it was firing up automatically unbeknownst to me?

 

I’ve also noticed that, at one point, I had two instances of Teams installed on the computer.  One was simply called ‘MS Teams’, and the other was something like “MS Teams Classic (Work or School)” – the latter is what I still possess on the machine.

It occurred again this morning – more severe this time.  “IRQL_Not_Less_Or_Equal”, seemingly related to a MS Teams boot.  I EXPLICITLY turned off auto-startup for MS Teams, yet it booted up on its own this morning for some reason.  When the window popped up, the machine crashed.  I had Outlook opening simultaneously, but did not see any syncing that may have correlated to the crash.

 

Upon restart, I received consecutive blue screens before even reaching the login screen. 

 

Eventually I was prompted with a windows screen asking me if I wanted to restart again, go to safe mode, inspect other options, etc.  I selected basic restart and ultimately it worked and I’m ok now. 

 

Questions/comments are:

1) Why would MS Teams auto launch when I know 10000% that I had it turned off?

2) Issues seem to be related to MS Office products, Outlook and MS Teams specifically.

3) Another culprit could be my dock and its drivers.  I know I’ve had these issues while not connected to it, but I don’t want to rule it out completely.

4) Today was one of the more severe experiences I’ve had in a while.  Is there any update on a potential action plan?  What about my comments yesterday regarding weird/duplicate versions of MS Teams?

Adding to the blue-screen log/drama.  Today was pretty severe.  Had a handful in sequence so I didn’t grab the error codes.

Wondering if it had to do with the new MS Teams.  It installed itself on Friday, and auto set itself to autoboot it seems (I’ve since turned that off once I got logged on).  Once again it looks like multiple MS teams versions are on this machine – Classic and the new one.

 

  1. Turned on computer, dock plugged in.  Got to log-in screen but nothing was responsive – keyboard, etc.
  2. Manually forced a restart via the power button.
  3. Restarted, was able to log in, MS Teams autobooted and function began, then after about 30 seconds a blue screen emerged.
  4. The machine restarted, and upon reboot another blue-screen emerged before the log-in screen came up.
  5. It tried to boot into a safe mode/bios debug mode automatically; I cancelled and did a regular restart as I’ve documented in this thread previously.
  6. Startup was successful, now functioning normally.

Another blue screen this morning - “IRQL_Not_Less_Or_Equal”.

 

The failure occurred while launching MS Teams and Outlook.  I did both somewhat simultaneously, although my feeling is that the crash corresponded to the Outlook calendar.  It occurred right as I received pop-up notifications about upcoming meetings.  I watched the sync status on the bottom, and I didn’t see anything that corresponded to a specific folder or action; I only noted what I mentioned above about the calendar/pop-up. 

 

Upon restart, the computer froze on the log-in screen after I typed in my password. 

A forced power-button restart was required.

 

After this, I began to function normally.

Another blue screen this morning, although I did not catch the error code.

 

This time, I made sure to take it slow and open one app at time.  MS Teams did not auto launch, so I launched outlook (manually) first.  It began doing its syncing and while going through that process, it crashed.  I’m admittedly guessing as to what point it crashed (I feel like it was calendar related as all of the new emails looked to have populated), but for what it is worth, Outlook was the only application open when it happened. 

 

I also waited a good 30 seconds or more before clicking anything to ensure DropBox, OneDrive, and any/all background apps, were successfully loaded and stable before engaging a new one.

3 blue-screens this morning, 2 of which occurred during boot-up either before the log-in screen, or immediately after hitting enter to log-in.

The 3rd failure occurred when I booted up MS Teams and joined a meeting. 

 

This is becoming unwieldy.  Can we elevate the priority of some of the corrective actions we’ve discussed?  Has there been any response from Microsoft on the tickets (Re: Windows 11 compatibility with higher-end hardware)?  I’m worried that, especially after today’s experience, my associations of the issues with MS Office software may be incorrect.

 

If we can also confirm a date for the server install, that would be appreciated.

The Bios is current ver # 321 as seen from Start-->Search --> type System Information

 

A question is the Virtual Pet Installed ? lol ??

 

Not much in the DxDiagx86 file : RADAR_PRE_LEAK_64        MSInfo32 : shows crashes : A lot of Microsoft Edge. It is trying to update. It Should be Updated from the 3 Dot Menu__>Help-->About (more or less)  It may not Update.  from Start-->Search --> type Troubleshooter and see if a Edge on is there. If not try the Update one.

Most everything should be turned off as it is full of junk and IMHO a security issue.

 

from Start-->Search --> type   Power will see Options click Then change this : Turn Off Hard Disk after: (On AC Power): 30 sec to never. (it's a SSD no moving parts. or change to some hour or 2 or 20 mins but never that low)

I will work through your comments in order:

 

1)  Noted on correct laptop nomenclature. Thank you :)

2)  I am a contractor to an engineering company.  I own the machine.  They have an IT Dept. and to their credit, they've been trying, but truthfully I think this is above them.  A fellow engineering friend in the motorsports community recommended me to this forum, so here I am :)

3)  Correct; BIOS Version/Date as per 'System Information' is G634JZ.321.  The date states 10/24/2023, but ironically, the machine (Asus Utility) forced a BIOS update last night (cited V321).   It completed successfully and everything was smooth afterwards, but this morning, all hell broke loose and I couldn't even get the machine to boot.  I ended up doing a recovery which allowed me to get back into Windows, but admittedly I neglected to document the date/instance it recovered to.

4)  If I have a virtual pet, I didn't install it lol.  I believe it came native with silly Asus adware.  Thought I uninstalled it, but perhaps something lingered, or I neglected to after the last fresh windows install to try and get to the bottom of these problems.  

5)  Microsoft Edge - Checking from within the app (Help --> About) states that I am fully up to date.  Edge is not present as an option via the troubleshooter menu in System via search.

6)  I have set the Hard Disk (SSD) to NEVER turn off while on AC Power.  On battery power it was increased to 10 minutes.  Let me know if I should revise further/differently.

 

 

About t start a MemTest86 run on the machine...

 

Note that immediately upon purchase, I added a 2nd SSD in the available slot, and increased the RAM to 64 GB from 32 GB.  This was done for me by the vendor/retail outlet I purchased from, but wondering if they ****ed up and there is a compatibility  issue.

3) is ok, you had a early release installed this is a up date to that same version since on a plan and part of being connected.  (Can update things through ArmouryCrate if want to check for your self.

 

4) No, was a LOL as per my signiture and ASUS knowledge. Mine is a rock and roll laptop no pets offered haha | Not a problem.

 

Will Cont.... Post #5 and give you the solution to the video issue.  But ya, do not bother with memtest with 64 gig will take a year.. :=}

No it looks good. is not unusual and once provide you the info you can always do so yourself if believe it is video related.

 

So my back needs a rest.    back in 20 with a 10min solution (I hope)

If you look above in my first reply documenting the various instances of BSOD, I mention a USB-C docking station and associated drivers.  Drivers are installed and dock is used daily; has been suspected to be a contributor.

I do not watch a lot of videos, but I do take a large number of conference calls via MS Teams.  Although, the faults rarely (if ever) happen during a video intensive exercise.  It's maybe happened once...  As mentioned, the crashes originally began when I'd first open MS Outlook or MS Teams and syncing would ensue (or so it would seem).  Couldn't confirm absolutely if there was some influence to the aforementioned dock, as the issue would not follow (dock plugged in vs. not).  

 

All NVIDIA drivers are up to date, while I have also done SFC /SCANNOW multiple times over the past few weeks and have never returned an issue.  Always reports clean...  Happy to try again though.

So save the driver as now you will do this : download DDU https://www.guru3d.com/download/display-driver-uninstaller-download/ (note, one time as there will be another version so need to go to the main page and look for it)  Follow the Instructions of removing your Video Card Drivers.

 

After then install the downloaded NVIDIA Drivers and test.

 

With luck someone will be by to read the Stack of all the Dump Files and see if there is something that is causing the issue that I am not seeing as not a Dump Expert :=}

As mentioned My suggestion here is just temporary and when getting Updates of Video Drivers you get updates.  So with DDU removing all drivers and the ones that maybe corrupted.  (Note SFC only fixes system files) 

 

A Dump Expert should get to your Dump Files which I glanced at. and figure out what may need further fixing. (midnight pacific time or there about)

Since there has been a Lot of System Damage it maybe best to do this 1st or just do when can.

 

from START | type cmd.exe into the start search box | RIGHT-click on cmd.exe | select "Run as Administrator" then Copy/Paste --> :DISM /Online /Cleanup-Image /RestoreHealth command then Enterif it finds problems then reboot. (Completed Successfully : means reboot)

& then do this again.

After : please Open another Admin Command Prompt and please run Copy/Paste --> SFC /SCANNOW into the Dos window or Powershell and Enter.  Reboot after.

 

The 1st goes up to Microsoft for new files if any are found to be corrupted. The 2nd will then check it self and update it self and fix issues if found with the files downloaded by DISM.

Thank you again for all of your help this evening.  

FYI, I am located in Toronto, EST time-zone.

 

1)  Western Digital HDD Utility was installed. There was a firmware update, so this was completed and updated.  Note that this is indeed the secondary drive that I added.

 

2)  Samsung Magician was also installed for the native drive, but no updates were found.

 

3)  DDU was downloaded, installed and utilized.  I ran this utility in safe-mode as recommended.  I imagine it's idiot proof and that I completed the task successfully, but I have never used such a tool before so there is a small percentage chance that I may have goofed on something.

 

4)  The latest NVIDIA drivers were installed (release date of November 14, 2023), and I ensured they were slated for Windows 11 and a Laptop.  In Geforce Experience, I checked if there were any updates available, and again, everything was reported as up to date.  

 

5)  DISM /Online /Cleanup-Image /RestoreHealth was then run, as recommended (note that it hung at 62.3% for quite a while).  When complete, I restarted the computer, and ran the same command a second time. 
 

6)  Once step 5 above was complete, I restarted the computer again and ran SFC /SCANNOW.  Again, I restarted the computer and ran this command a 2nd time (perhaps redundant).
 

Note:  In the case of both steps 5 and 6 above, all runs were completed successfully.   

 

 

I suppose I'm now in test mode.  I will operate as usual over the coming days and report any more faults or BSOD.  I will run/provide additional 'sysnativefilecollectionapp.zip' files as warranted as well.

In parallel, I will also stay tuned in hopes that a 'Dump Expert' has an opportunity to review my files and provide some feedback.

Hi, How did you get to Geforce Experience from DDU (using it properly) when I provided a link and asked you to go there.  The Driver Installer as laid out in Post # 9 was from the 15th version 546.17.  Or did you do a typo ?

The object is to always get the newest driver.  As 1 below may still cause issues.   It is the advise given by the expert named below.

 

5} Dism always hangs for a while at around that % :=} ,      "all runs were completed successfully" - Perfect ,  run a 2nd time (perhaps redundant) - yes  lol

 

You are in surviving test mode until the BSOD Kernel Dump Expert  @ubuysa takes a look at the rest of the Dump Files.      I only cleared you up of the obvious (to me) to be stable and fix System Files.  With the amount of crashing were saying and number of Dump Files Dism was warranted.      Something different may have started the problem and that is what will be looked at stuff over my head :=}

 

As this is a ASUS please in BIOS turn off ASUS Fast Boot for the same reasons as below.

If this is a PC owned by your employer and you have an IT department then I'm personally uncomfortable trying to fix this outside of your IT department. Therefore everything I suggest in this thread is advisory only and primarily for you to show your IT department, not for you to act upon. I've not read all your emails, they don;t seem to help much.

 

You uploaded five dumps but your System log records 27 BSOD, so where are the other dumps?

 

Three of the dumps fail during a graphics operation, two of the dumps reference nvlddmkm.sys, the Nvidia graphics driver, which is expected. One of these graphics failures however, references the Intel iGPU driver igdkmdn64.sys. It's most unusual to be using both the iGPU and the dGPU at the same time, yet you clearly are; the nvlddmkm.sys and igdkmdn64.sys dumps are timestamped less than 10 minutes apart.

 

What sort of graphics setup are you using?

 

Both graphics drivers are likely to have been superseded. The Intel driver dates from June 2023 and the Nvidia driver dates from March 2023. I would suggest looking for driver updates for both.

 

One of the dumps (the 0xA IRQL_NOT_LESS_OR_EQUAL) can probably be laid at the door of the ngscan.sys driver, it's the only third-party driver on the call stack leading up to the bugcheck. This appears to be a driver for an Acronis product. The call stack in this dump also references ntfs.sys, the Windows file system driver, so some sort of file storage access was in progress and one might expect an Acronis product to be involved there.

 

The ngscan.sys driver you have installed is old, dating from March 2021, so it might be worth looking for any update to whatever Acronis products you use.

 

The fifth dump is inconclusive. The bugcheck indicates that a driver messed up the count of APCs (asynchronous procedure calls) but the bugcheck happens as soon as the kernel is entered. In addition the function that fouled-up was a user-mode function, so this could well be related to the Acronis product which has already been the likely cause of one of your BSODs but it's not possible to say with any certainty.

 

It would be useful to see any other dumps you may have, it would be a pity if they have been deleted because the more dumps we have the better the picture we can build.

 

I would talk again to your IT department and query  the Acronis product version because that certainly associated with one dump. I would also query the graphics setup you have because it's not usual to see both GPUs in use at the same time, and look for updates to both graphics drivers.

@Pkshadow @ubuysa - There may have been a misunderstanding.  This machine is owned and has been paid for by me exclusively.  I am an engineering consultant/contractor for a motorsports team and engineering firm which enlists their own 3rd party IT service for a monthly retainer.  I am outside of that ecosystem entirely as I am not their 'employee', however as a courtesy and given that we are all friends, they have been nice enough to try and assist over the past number of months, but to no avail.  We are 100% free to proceed as we see fit.  Treat this as an interaction with an individual consumer.

With respect to the DDU exercise, the application ran (while in safe-mode) and presumably removed the previous video-card drivers.  The machine automatically restarted, after which I installed the new drivers which I attained from the link you provided.  Specifically, I navigated here:  https://www.techspot.com/drivers/downloadnow/17702/?evp=7225d54f0e64401b0f20f12b97b2b647&file=25559

 

I clicked on the downloaded file attained from the above link, and it installed the entire NVIDIA suite, including Gforce Experience.

 

If I've misunderstood the process and did something incorrectly, I'm happy to repeat with some added clarity.

The dumps I've provided are unfortunately all I have.  This has been going on for months (I did a fresh Windows install in September - things were OK for a few weeks, then began to re-emerge).  The reason there isn't a longer record in the reports I provided is (I believe) due to the fact that I was forced to do a system recovery upon startup yesterday as the system would not boot.  You would know better than I would, but I suspect this took me back to some previous place in time, eliminating any logs between then and now.

 

As for graphics, the machine in question here replaced a MSI unit (also a laptop) that I had for many years.  It too had a NVIDIA card, in addition to something Intel native to the motherboard.  I don't have nearly enough experience to comment absolutely, but that computer also leveraged both GPU's simultaneously to achieve performance.  That previous machine was retired because the NVIDIA GPU physically died.  In any case, it seems to me in my experience that this type of architecture and functional dynamic is commonplace in laptop land.  I may be completely incorrect and am happy if someone with more knowledge clarifies.  

 

As for my graphics setup, I am not doing any gaming or anything intensive.  This is a work laptop for engineering activities.  When not on the road, I link the machine via USB-C to the VisionTek dock outlined earlier in the thread (link to drivers also provided).  It connects me to 2 additional 'ThinkVision' monitors, allowing me to run 3 screens in total including the laptop screen.  I run software's like SolidWorks, but these issues have never occurred doing anything I would consider graphics intensive (never had a crash with SolidWorks, for example).  Until recently (startup issues, etc.), the crashes would only occur when launching Outlook or Teams, as examples (a few minutes after logging in, seemingly during the 'sync' process).

 

As for the Intel drivers, if I inspect via Device Manager, I am told that all drivers are currently up to date.

With respect to Acronis, I have 'Acronis True Image' installed on this PC, but it is not licensed.  I have Macrium as well for proper images which I use.  

Acronis is a bit of a long story...  The company I am contracted to has a NAS to store motorsport video and data.  I have it mapped as a network drive, but in order to gain access, I was told by the aforementioned contracted IT dept. that I needed 'Acronis True Image' installed as a navigation utility (eventhough I never actually enter the software - it's as if it does something in the background).  I can try to update it, or alternatively, blow it away entirely, but because I don't actually have a licenesed usable version, I'm not sure updating is a possibility.  

 

2)  Would you recommend eliminating Acronis entirely?  Perhaps I can try to map to the NAS independently without this software installed?

If it needs to be retained, in light of the odd situation I've tried to explain, do you have a brainstorm of how best to proceed?

 

With respect to the 5th dump, the only other thing I could brainstorm is maybe something related to DropBox or OneDrive, both of which I leverage heavily for my work (if not Acronis related, but I'm talking out of my @$$).

 

I will continue to try and operate 'as normal', and if crashes occur, continue to log and resubmit to you gents.
